Upload mandatory document
- ID copy of policy owner ( if has not submitted before)
- Doctor / Hospital issued medical cost breakdown and receipt
- Reports ( Lab test , Ultra Sound , X-ray , CT Scan , MRI Scan, Pathology report)
Upload supplementary document (if applicable)
- Discharge Summary issued by the government hospitals
- Sick-leave certificate with cause of illness listed
- Police Station report and/or Traffic accident report
- Other insurers’ claim reimbursement breakdown
- Under special circumstances, we may asked for further supporting document to process the claim application
- Submit eClaims for my client
